Submit an application online or in person
Complete a release of information form.
If you receive a denial letter, contact Terri for help to resubmit
If approved:
- Send Myranda Carter your approval letter in order to register your child(ren).
- Note that there may be a copay for our program.
- Myranda Carter will direct you through the registration process.
