Activities Requiring Pre-Approval

Please note that all forms on this page are fillable forms and should be downloaded to your computer to complete.

Some professional development activities require preapproval by the LPDC (see LPDC Handbook) in order to count towards license renewal. To receive pre-approval and CEU credit you complete the following forms.

Use to document hours spent to complete approved project

Contact Hour Documentation

Some activities do not issue a contact hour certificate. In those cases, please complete one of the following forms and attach any available documentation (e.g. registration form, agenda) to document the amount of time spent.
Use to document webinars that do not provide a contact hour certificate. If available, attach documentation of registration/attendance.
Use to document professional development that does not issue a contact hour certificate. If possible, attach an agenda to verify the time attended.
Use to document mentoring of a Year 1 or Year 2 Resident Educator. An activity log should accompany this form to document hours spent on mentoring.

Online Learning Guidelines

During the Coronavirus (COVID-19) emergency, the LPDC adopted procedures so that educators could earn credit for activities related to online learning. Only work that was completed from March 13, 2020 to June 17, 2021 will count towards license renewal under this specific procedure. 

CEU Conversion Chart

Use the CEU Conversion Chart to convert contact hours, semester hours, and quarter hours to CEUs. 18 CEUs are required for license renewal.

Use the Vector Learning Conversion Chart to convert time spent completing Vector Learning courses to contact hours. Also, see the Vector Learning Summary Form.

Summary Forms

It is strongly encouraged that each individual keep track of their professional development activities as they are completed in order to submit their activities to the LPDC in an organized manner when it is time to renew their license. For those purposes, use of the following forms on an ongoing basis is strongly encouraged.
